eSOURCE - CROWNWEB Alpha Testing

The CROWNWeb Alpha 2 Release provides the overall foundation for CROWNWeb Release 1.0. The following features are supported in this release:

A user will be able to search the provider records for all providers.
A user will be able to search, add, save, edit, and submit a patient record. As each patient is added, that patient will be associated with a facility and a start date of dialysis at that facility.
A user will be able to enter information associated with the 2728 form only for initial ESRD hemodialysis patients (without training or transplant).
A user will be able to enter a 2746 form for a patient.
A user will be able to print the 2746 and 2728 forms.
